PRK is the acronym for photorefractive keratectomy which is a procedure involving the removal of the surface layer of the cornea (epithelium) by gentle scraping and use of a computer-controlled excimer laser to reshape the stroma.
Presbyopia is part of the normal process of aging. As a person becomes older, one begins to lose the flexibility of the lens of the eye which limits the ability of the eye to change its point of focus from distance to near.
Trephine - (truh-FEEN) A surgical tool with serrated edges, used to cut circular pieces. It’s used in a LASEK procedure instead of a microkeratome to cut the corneal flap.
DALLAS LASIK - Dr William A. Boothe: It has a suction ring that keeps your eye pressurized, which is important in LASIK for allowing the surgeon to cut the corneal flap. Dr. Boothe, the surgeon will use an ink marker to mark the cornea before the flap is created. The flap is then created with either a microkeratome or with a laser, depending on the surgeon's preference.

CrystaLens - A type of intraocular lens which changes focus between close and distant objects. More information.
Crystalline lens - The eye’s natural lens, which is behind the iris and in a healthy eye is completely clear. Light passes through it and is refracted by the lens to focus on the retina. Tiny muscles attached to the lens change its convexity according to where the eye is focused. As we age, it can start to become cloudy (cataracts), impairing our vision.

BOOTHE - LASIK SURGERY: There is also Pseudoesotropia, which looks like esotropia but is a temporary condition where a young child’s eyes haven’t yet grown the white part next to the nose. The bridge of the nose appears wide and the eyes appear to be both turned inward. This resolves itself as the child’s face develops.
Misalignment of the eyes in childhood should be corrected as soon as possible, so that binocular vision (the brain’s ability to use both eyes together) can develop and amblyopia can be avoided. Surgery may be needed; or glasses, an eye patch, or other types of treatment.

LASIK (Laser in-situ Keratomileusis) is used to correct or reduce moderate to high levels of nearsightedness (myopia), astigmatism and farsightedness (hyperopia). The traditional LASIK procedure is relatively simple: a keratome is used to gently create a flap in the cornea. In IntraLASIK, the flap is created by a computer-controlled, precision laser.
The flap is lifted and an excimer laser is then used to shape the remaining tissue. The flap is then carefully set back into place, forming an occlusive seal by osmopic pressure. Osmopic pressure holds the flap in place and allows it to heal very quickly, without stitches!
Laser surgery for eyes is also recommended for glaucoma patients. Glaucoma results from poor drainage of aqueous fluids in the eye. Dr. Boothe, the result is a build- up of pressure that can press against the optic nerves. This can lead to serious complications especially if the optic nerves are damaged beyond repair. Laser beams can be directed into the area where this fluid accumulates to widen the angle through which it flows.
Dr. Boothe, in the case of glaucoma caused by the construction of new and more blood vessels, laser can be used to destroy these new and often poorly formed blood vessels so aqueous fluids can easily flow.

limbus -
The visible borderline between the clear window (cornea) and the white globe (sclera) of the eye. The conjungtival layer which covers the globe also joins at the limbus. (see illustration x)
masks - Masks are used in PRK surgery to modify the removal of surface tissue by the laser. PRK surgery involves the removal of or fashioning of concave (myopic) or convex (hyperopic) lenses on the corneal surface. Most refractive errors also include some element of astigmatic irregularity. Most excimer laser machines employ different kinds of masks to customize, refine and smooth the corneal surface. Many machines have an internal constricting diaphragm (like a camera f-stop mechanism) as an internal mask for use in treating Myopia. One manufacturer uses an extra material applied unto the cornea (an ablatable or destroyable mask) for treating astigmatism. Another manufacturer uses a different shaped metal mask for treatment of hyperopia or myopia. Variable rotation of the masks is used to deal with astigmatism
astigmatism - A refractive abnormality where light is not focused to a point on the retina. This is often caused by the cornea or front window of the eye being elliptical (shaped more like an egg) rather than spherical (shaped like an orange). Refractive correction includes a lens which has focusing power in one axis and no power in the opposite axis. The amount of astigmatism you have will appear in the second number of most glasses prescriptions ie - 4.00 -1.00 x 30°, or -5.00 +1.00 x 120. In both of the above cases you would have 1.00 Diopter of astigmatism.

WILLIAM BOOTHE - EYE SURGERY: Fuch’s Dystrophy - A progressive, inherited eye disease (dystrophy) in which the cornea loses cells from the endothelium which normally remove fluids and impurities from the eye. Without those cells, the eye retains too much fluid and begins to swell. By changing the cornea’s curvature, this makes vision blurry, especially first thing in the morning, since while the eyes are closed in sleep, no moisture can evaporate from them. It causes other symptoms, e.g. blisters, light sensitivity, pain, and decreased depth perception. There is no cure, but there are some ways to minimize symptoms. A corneal transplant eventually becomes necessary. Also called Endothelial Dystrophy.

Convex Lens - Lenses with the opposite curvature to concave lenses. They bulge outwards like a ball and are used to correct hyperopia and presbyopia.
Cornea - (KOR-nee-uh) The front clear part of the eye in front of the pupil and iris. It acts as a lens, refracting light rays as they enter the eye. The eye’s crystalline lens refracts them further to focus them on the retina at the back of the eye. The cornea also allows light to pass out through it, making the iris visible, the eye’s color.

DR BOOTHE A. - EYE SURGERY: diplopia - Double vision or seeing double usually with both eyes open as in binocular diplopia, but can be with only one eye as in monocular diplopia.
emmetropia - Perfect distance vision without the need of glasses.
endophthalmitis - An inflammation within the eye. Inflammations may be caused by organisms such as bacteria or may be sterile as in immune disorders. Endophthamistis usually is used to indicate an infectious disease as occasionally occurs as a complication of surgery.

Central Ablation Zone - The ablation zone is the area of the eye treated by the laser in LASIK surgery. Around it is the transition zone, where the treated area gradually merges with the original corneal surface that lies outside the ablation zone.
Central Island - An area of the laser-treated part of the cornea which is erroneously not treated, so that its level remains microscopically higher than the surrounding treated surface. The term island describes its appearance. It causes diplopia (double vision).

It's as if the refraction is "ground in" to the middle layers of the cornea! The results are often dramatic and very rapid, with most patients seeing well enough to drive a car without correction the very next day. However, the best post-operative visual acuity may not be obtained until 2 to 3 weeks, or in some cases, even a few months, after the procedure.

retina - The light sensitive nerve layer which converts light images into electrical signals for transmission to the brain. The retina can be compared to the film of a camera. See Peter's diagram.
undercorrection - A complication of PRK where the expected amount of correction is less than desired. See glossary entry "regression". Undercorrection occurs in the occasional person where healing occurs more vigorously than predicted by the laser computer program. Undercorrection can also occur in the uncommon event that the laser is improperly calibrated.

keratectomy - Surgical excision (removal) of any portion of the cornea. In a penetrating keratectomy (or PK) a button-like full thickness segment of the cornea is removed and replaced with a donor cornea from another person.
keratotomy - Surgical incision of the cornea as in radial keratotomy ie. a radially or clock hour aligned incision in the cornea.

It means that there are more number of lasik facilities available here, and therefore, more choice of facilities and more chances of getting the best care. There are quite a few renowned optometrists who have their practice in New York.
